Our Neighborhood Health initiative is designed to bring more health prevention services and support directly to the people and communities we serve. Here’s how it works
Our mobile “CareVans” visit and operate daytime health clinics at specifically chosen locations every week. They offer a variety of health services including screenings, vaccines, testing, medical assessments, referrals, education and support.
A partnership
Neighborhood Health was developed in collaboration with trusted community partners throughout the state. These groups and individuals helped determine the health needs and priorities for their residents and communities. These innovative health clinics will be adaptable, flexible, and open to feedback to ensure access to needed services and programs.

Our Neighborhood Health clinics...

Our CareVan Clinics offer the following services:
- Address various urgent needs (not limited to):
- Common cold/flu
- Stomach aches
- Ear aches/infections and foreign body removal
- Family planning
- Abscess incision and drainage
- Minor burns, lacerations, sprains and fractures
- Rashes and insect bites
- Corneal abrasions
- Medical Screenings:
- Blood pressure, Blood sugar
- Lab tests:
- A1C, Cholesterol, Hepatitis C, Hemoglobin, HIV
- Chemistry Panel (Liver and Kidney function)
- Referrals, Follow-up Care:
- Social Services, Behavioral Health, Primary Care and Specialty Care
- Immunizations:
- Tetanus (Tdap)
- Pneumonia
- Shingles
- Hepatitis B
- Meningitis
- Gardasil
- Polio
- Flu (seasonal)
- MMR (Measles, Mumps and Rubella)
