Before ordering a sleep study, please refer your patient to a sleep specialist for a consultation. This allows us to determine the most appropriate type of sleep study specific to each patient.
The Ayer Neuroscience Institute has sleep care locations across the state. Please choose the location that is the most convenient for your patient.
How to Make a Referral
For Hartford HealthCare providers, please select “Ambulatory Referral to Sleep Medicine” in Epic and choose the appropriate location.
For non-Hartford HealthCare providers, a paper referral form is available. Please contact the desired location for more information.
