Connecticut Advance Care Planning Guidelines

Advance directives in Connecticut must follow certain state requirements to help ensure your wishes are clearly documented and legally recognized.

Completing an advance directive doesn’t have to be complicated. In most cases, you can fill out the forms yourself without a lawyer.

To complete an advance directive, you’ll need to:

  • Fill out the appropriate forms
  • Sign in front of two witnesses
  • Make sure the person you choose as your healthcare representative is not one of your witnesses

After you complete your forms

Once your advance directive is signed, it’ important to share it with the people who may need it.

Be sure to:

  • Give a copy to your doctor
  • Share a copy with your healthcare representative
  • Let family members or trusted loved ones know where to find it
  • Bring a copy to medical appointments or hospital visits when appropriate

Your wishes may change over time, and your advance directive can change with them. You can update or revoke your advance directive at any time.

It’s a good idea to review your documents after a major life change, a new diagnosis or a change in the person you want to make decisions for you.
