Hospice care is mostly provided through Hartford HealthCare at Home at the patients residence, but services may also be arranged at any of Hartford HealthCare's hospitals, nursing homes or assisted living facilities.
Hospice services include Nursing, Medical Social Services, Physician Services, Hospice Aide/Homemaker Services, Volunteer Services and Spiritual, Dietary and Bereavement Counseling.

The hospice team will help patients and their families make informed decisions about caregiving and guide families through the support services when hospice care is required.
